Transitional Housing Client Move-Out

Please complete this form and the pre move-out inspection checklist, then upload it to the ticket system. Case Managers must give one week's notice for the move-out, and two week’s notice if the client requires additional support moving into their new residence.

Two (2) days prior to the move-out date, Operations will complete a move-out inspection. 

Operations will conduct a quick pre move-out inspection, to ensure that the room is clean and in functional condition. The clients belonging should be mostly boxes at this stage. 

Pre-Move out inspection Date and Results: 

 

On move-out day, the Case Manager will hand the key back to Operations.

The day keys are returned to Operations is considered the final move-out date. A $25 lost key fee will be assessed to client if keys are not returned, and SMC reserves the right to prorate room rental fees until keys are returned.
